Parent Handbook - This handbook will provide detailed information regarding our program.

Rain Policy - This document will provide information regarding procedures in the event of rain delays or weather cancellations.
Registration Form - This form is required to register with all Associations
Physical- This form may be used by the Doctor performing the participants physical examination. All forms must be signed by Doctor and his name must be legibly printed or stamped.
Scholastic Application - 2 page form used to evaluate the participant with regards to the Tucson Youth Football and Spirit Federation Scholarship program.
Participant and Parent Code of Conduct - Mandatory form which must be sign by both the Participant and a parent/guardian.
Coach and Staff Code of Conduct- Mandatory form for all Coaches, Assistant Coaches, Equipment Managers, Business Managers, and any other team staff.
Registration Sign-Up Sheet- Used by each Association to submit new registration information to the Federation
School Agreement - This form is to be completed by those participants in the Tucson Youth Football and Spirit Federation program that have not met the Scholastic Eligibility requirement of 70 percent and/or 2.0 Grade Point Average (GPA). The Participants School Administration must complete this form.
Tracking Agreement- Only used if the Participants School Administration requires the use of tracking. Both the Participant and a parent/guardian must sign this form in agreement that the participant must be tracked weekly for acceptable participation in their classes.
Tracking Form - Used by those Participants that are required by their school to be under our tracking program. MUST be completed weekly for the Participant to be eligible to participate in a game or competition.
Cast Authorization - Must be used for a Participant to be eligible for practice, games, or competitions.
Mandatory Play Roster - Used by each football team for every game to demonstrate they have complied with the rules regarding Mandatory Play.
Injury Form- This form is to be completed by the attending Medical person (with assistance from the Team Business Manager) for any injury that requires referral to a physician or hospital or immediate medical treatment. This report must be signed by both the attending Medical person and the Team Business Manager. All injuries must be reported with in three (3) days.
